Patient Experience Coordinator, Nursing Administration, Full-time Days, Clarksville

Job Summary Applies the nursing process to plan and manage the care of complex patients and programs in a comprehensive manner. Provides consultative services and actively seeks opportunities to collaborate with the health care team. Recognizes complex learning needs and consistently demonstrates effective patient and staff education. Demonstrates and role models effective communication to coordinate patient and family-centered care. Facilitates follow up care and/or refers patients to the appropriate next level or site of care. Serves in an advocacy role on behalf of patients, families, staff, and caregivers to ensure safe, equitable healthcare provision. Ensures evidence-based care and continuously monitors patient and program outcomes. Engages in learning activities according to Board of Nursing standards. Key Responsibilities Collaborates with interdisciplinary team, attending physicians, families and other professionals to formulate and initiate specialty norms and maintain collaborative practice agreements. Utilizes advanced clinical knowledge to provide or guide safe and effective continuum of care. Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care and program needs across the continuum. Analyzes comprehensive information pertinent to the patient’s care and management to determine nursing diagnosis. Continually evaluates patient and program progress towards goals and applies appropriate interventions to ensure effective outcomes. Employs strategies to promote health and a safe and healthy environment. Technical Capabilities Nursing Patient Assessment & Evaluation (Intermediate): Demonstrates mastery of patient assessments and evaluations in practical applications of a difficult nature. Conducts primary care patient interviews and physical examinations. Care Planning (Intermediate): Demonstrates ability to anticipate and plan for complex resource needs, identify discrete tasks, set priorities, schedule activities, meet deadlines, and organize work. Nursing Patient Education (Intermediate): Demonstrates mastery of patient education in practical applications of a difficult nature. Evidence-Based Practice (Advanced): Recognizes, implements, and evaluates practice changes based on published research. Qualifications Licensure: Registered Nurse (LIC). Experience: 3+ years of relevant work experience. Education: Bachelor’s degree in nursing or related field.
