(M-F), with 4 individual weekend shift requirements in a 6-week period. Shift: Days Location: East Orlando campus The Role You’ll Contribute The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources and qualify for community assistance from a variety of special assistance programs and agencies Assistance with transitions of care or discharge planning. Offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, RN Care Managers, staff nurses, and other members Education Master's MSW [Required] Work Experience 2+ social work [Required] 2+ care management experience [Preferred] Physical Requirements (Please click the link below to view work requirements) Physical Requirements - Pay Range $23.94 - $44.53 Background Screening Requirement (Florida Law) Certain positions are subject to Florida Level 2 background screening , including fingerprinting, as required by state law.
