Job Description
TITLE: Speech-Language Pathologist

SCHEDULE: 8 hours

ASSIGNMENT DETAILS:
- Contract Length: 13 weeks
- Guaranteed Hours: 40

REQUIREMENTS:
- Current Certificate of Clinical Competence (CCC) in Speech-Language Pathology from the American Speech-Language-Hearing Association (ASHA) or CCC eligible.
- Current state license as applicable.
- Proof of current CPR certification.
- Current driver's license.
- Reliable transportation and insurance.

ROLES & RESPONSIBILITIES:
- Conduct evaluations and assess the home environment to identify equipment needs related to speech/language function.
- Identify functional speech defects and establish a Plan of Care (POC) to enhance the patient's functionality.
- Administer speech therapy programs utilizing specialized therapeutic techniques and/or equipment.
- Recommend communication devices/aids as indicated.
- Collaborate with referring physicians and clinical staff to ensure coordinated care contributes to the total Plan of Care (POC).
- Document patient care progress and the continuing need for treatment.
- Educate patients and families/caregivers on tasks and resources that support continued functional independence.
- Identify needs for additional services and refer as appropriate.
- May supervise home health aides in procedures related to the speech-language therapy Plan of Care (POC).
- Participate in case conferences and meetings to ensure coordinated care.
