Clinical Case Manager: Recovery Care & Discharge Planning

Innova Recovery Center seeks a Clinical Case Manager to coordinate client care across Residential, PHP, IOP, and OP programs. You will maintain an outpatient caseload, provide individual therapy as assigned, and facilitate therapeutic groups while collaborating with multidisciplinary teams. Responsibilities include discharge planning, resource coordination, and ensuring continuity of care across locations. A Bachelor's in a behavioral health field and LMSW or similar licensure is preferred; #J-18808-Ljbffr Priority Dispatch Corp.
