Schedule: 37.5 hours per week

Assignment Details:

• Caseload: typically 1215 patients, can be up to 20
• Patient ages: K12 with a small early childhood caseload

Roles & Responsibilities:

• Provide direct patient care for a diverse caseload of pediatric patients.
• Develop and implement individualized care plans based on patient needs.
• Monitor and assess patient progress, adapting care strategies as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Educate patients and families about health management and treatment options.
• Maintain accurate and up-to-date patient records.
