Hybrid Nurse Case Manager — Foster Care & Care Coordination
Little Flower Children and Family Services of New York
Little Flower Children and Family Services of New York is seeking a Nurse Case Manager to handle the full patient care cycle for children in foster care. This full-time hybrid position requires 3 days in the office at their Brooklyn location. The ideal candidate will coordinate medical and mental health care, develop tailored care plans, and communicate with care teams to foster optimal health outcomes for children.
