Hybrid Nurse Case Manager — Foster Care & Care Coordination

Little Flower Children and Family Services of New York is seeking a Nurse Case Manager to handle the full patient care cycle for children in foster care. This full-time hybrid position requires 3 days in the office at their Brooklyn location.
