Family Services Coordinator

Family Services Coordinator
As our Family Services Coordinator, you will conduct comprehensive family assessments, develop individualized family plans, and provide crucial in-home and on-site case management services to enhance family stability and support early childhood education. Every day you will manage a caseload of 35-40 families, offering family support, parenting education, and social services including referrals, advocacy, and services brokering, all while collaborating with a flexible team to ensure high customer satisfaction. To be successful in this role, you will demonstrate strong assessment and case management skills, a deep understanding of family support needs, excellent communication and advocacy abilities, and a commitment to empowering families and ensuring optimal outcomes for young children and pregnant women.

Founded in 1887, Dignity Health - California Hospital Medical Center is a 318-bed, acute care, nonprofit hospital located in downtown Los Angeles, California. Serving over 100,000 patients annually, the hospital offers a full complement of services including a Level II Trauma Center, Level III NICU, heart care, women's health, and a family birth center. In 2025, we opened our new patient tower, Grand Tower, featuring a dramatically expanded Emergency Department and Family Birth Center that will increase patient comfort and privacy.
Additionally, California Hospital Medical Center has been recognized as an LGBTQ+ Healthcare Equality Leader by the Human Rights Campaign Foundation.
