PUBLIC HEALTH NURSE SUPERVISOR

Position Summary The Public Health Nurse (PHN) Supervisor will establish, implement, monitor, and evaluate quality, performance, and patient safety of patients served by the Public Health Nursing department. The supervisor will ensure compliance with Joint Commission accreditation, government regulations, and the PHN Scope and Standards of Practice, coordinate services with a multidisciplinary care team, and oversee staff to positively impact high‑risk groups and contain communicable disease outbreaks. Essential Duties & Responsibilities Coordinate and participate in planning and delivery of health care services through collaboration with SMH providers, case management, tribal, state, and county agencies. Lead a multidisciplinary care team and implement care plans for high‑risk groups including foodborne illnesses, TB, STIs, vaccine‑preventable diseases, high‑risk elders, obstetric patients, and homebound patients. Conduct and coordinate epidemiological surveys, investigations, and surveillance of index cases and contacts; provide emergency preparedness support with local, tribal, state, county, and national programs. Conduct needs assessment surveys, identify community resources, gather community profile data, and prioritize health needs. Participate in quality assurance activities, evaluate PHN program activities, and improve quality patient‑care outcomes. Develop short‑ and long‑term program goals and objectives based on community needs assessment and National Healthy People initiatives. PHN Field Coverage Provide direct patient care and nursing services during home visits, including patient education and vital sign monitoring. Document patient care encounters in the Electronic Health Record in a timely manner using the SOAP format. Adhere to confidentiality, SMH policies, HIPAA/HITECH regulations, and document required forms, reports, and logs. Maintain vehicle and equipment; perform preventative maintenance and troubleshooting. Always wear proper PPE, including face mask, gloves, gown, isolation gown, NIOSH‑approved N95 or higher respirator, eye or face shield. Perform PPE donning and doffing safely and dispose of used PPE according to CDC guidelines. Conduct routine cleaning and decontamination of surfaces contaminated by communicable diseases. Perform other related duties as assigned. Qualifications Master of Science in Public Health Nursing or Community Health Nursing (MSN) with a current, unrestricted nursing license and at least two years of public health nursing and clinical experience and one year of supervisory experience. Or Bachelor of Science in Nursing (BSN) with a current, unrestricted nursing license and at least four years of public health nursing and clinical experience and one year of supervisory experience. Valid driver’s license with no restrictions. Valid American Heart Association (AHA) Basic Life Support (BLS) certification; maintain certification. Successful completion of the Employee Health Program requirements and background investigation. Knowledge, Abilities, Skills, & Certifications Demonstrated satisfactory performance in all prior employment. Knowledge of public health nursing scope and standards of practice. Strong interpersonal and organizational skills. High ethical standards with no complaints. Reliable and dependable; punctual with minimal absences. Strong customer service skills and telephone etiquette. Proficiency in Microsoft Office applications. Excellent communication skills, including ability to speak Navajo and/or familiarity with the Navajo Way. Physical Demands The employee is required to sit, use arms and hands to reach and grasp, talk, hear, and walk. Occasional standing, stooping, kneeling, crouching, crawling, and lifting up to 25 pounds is also required. Work Environment The work is generally performed in an office with a moderate noise level.
