Job Specifics and Requirements

• Location: Lake Charles, LA

• Schedule: Full-Time

• Education: Pharm.D.

• Experience: 1+ years preferred, but new graduates are highly encouraged to apply!

• Licensure: Must have (or be eligible to obtain) an open, active, and in good standing Louisiana State Pharmacist License.

• Pre-Employment Screening: Ability to successfully complete a background check, health screen, and drug screen.

Job Responsibilities

In this role, you will maintain productivity, quality standards, and legal compliance within a long-term care pharmacy setting. Your daily responsibilities will include:

• Reviewing and verifying medication orders for long-term care facility residents to ensure accuracy and appropriateness.

• Conducting comprehensive drug regimen reviews and identifying potential medication-related issues.

• Collaborating with physicians, nurses, and healthcare teams to optimize patient medication therapy.

• Monitoring for drug interactions, contraindications, and adverse effects.

• Ensuring compliance with federal and state regulations governing long-term care pharmacy operations.

• Providing consultant pharmacist services and recommendations to facility staff.

• Participating in interdisciplinary care planning meetings and providing clinical expertise.

• Maintaining accurate documentation and records in accordance with regulatory requirements.

• Educating healthcare staff on proper medication administration and storage procedures.

• Responding to medication-related inquiries and providing drug information support.

• Overseeing medication storage, handling, and disposal protocols within the facility.

• Conducting quality assurance activities and participating in continuous improvement initiatives.
