Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings while crafting personalized treatment plans tailored to individual needs
• Implement physical therapy techniques aimed at enhancing mobility, building strength, improving balance, and restoring function
• Engage with a multidisciplinary team including nurses, physicians, case managers, and other rehab specialists to drive successful health outcomes
• Guide and educate patients and their families on therapeutic objectives, discharge strategies, and effective home exercise practices
• Maintain detailed records of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
• Contribute to care team discussions and interdisciplinary conferences ensuring cohesive patient care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized and accredited program

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred but not mandatory
• Additional specialized certifications (neurological, geriatric, etc.) are advantageous but not required

Skills

• Previous experience is highly desirable
• Proficient in managing patients facing complex medical and mobility challenges
• Excellent communication and collaborative skills
