What You’ll Do

• Provide quality hospice care and patient assessments in accordance with clinical guidelines
• Coordinate and conduct patient visits, averaging 4 visits per day
• Collaborate with healthcare team to develop and implement care plans
• Document patient care and progress accurately and timely
• Educate patients and families about hospice services and treatment options
• Ensure compliance with all healthcare regulations and standards

What You Need

• Current NP license in Texas
• Experience in hospice or palliative care preferred
• Strong communication and patient management skills
• Ability to work independently and as part of a multidisciplinary team
