As part of our in‑house therapy model, you’ll collaborate closely with nursing, leadership, and other rehab team members to make a direct, positive impact on resident lives—every day. Key Responsibilities Perform comprehensive evaluations of residents’ physical conditions and functional abilities Develop and implement customized treatment plans using evidence-based practices Provide therapeutic interventions to improve mobility, balance, strength, and functional independence Monitor progress and adjust care plans in collaboration with the IDT Educate residents, caregivers, and families on mobility techniques and home exercises Ensure timely and accurate documentation in line with facility and regulatory standards Participate in team meetings and support an integrated care approach Adhere to all safety, compliance, and regulatory requirements Why Join Paradigm at Woodwind Lakes?
