Requirements:

• Licensed Clinical Social Worker (LCSW) in California
• One year of acute care experience required; two years preferred
• Familiarity with MediCal and Medicare
• Cerner experience strongly preferred

Roles & Responsibilities:

• Provide clinical social work services within an acute care setting
• Conduct assessments and develop appropriate treatment plans
• Collaborate with interdisciplinary teams to ensure holistic patient care
• Advocate for patients and families to access necessary resources and support
• Document clinical interventions and patient progress consistently
• Participate in discharge planning and follow-up care coordination
