Key Responsibilities\n

• Conduct thorough assessments of clients' mental, emotional, and social health to identify needs and establish individualized treatment plans.

• Implement and manage tailored support packages that empower clients to overcome dependencies and improve their overall well-being.

• Provide crisis intervention and emotional support during sessions, utilizing active listening and empathy as fundamental tools in promoting client safety and stability.

• Monitor and document clients’ progress, adjusting treatment plans as necessary based on evolving needs or circumstances.

• Engage in professional collaboration with multidisciplinary teams to ensure comprehensive care approaches are applied to each case.

• Participate in case conferences, providing insights and evidence-based recommendations to optimize client outcomes.

• Act as a liaison to other community services or resources, facilitating necessary referrals for additional support such as housing, financial assistance, or substance recovery programs.

• Maintain accurate and up-to-date records in compliance with confidentiality and ethical standards.

Patient Population / Client Focus\n

This position primarily involves working with clients who are experiencing severe stressors, including but not limited to terminal illness, end-of-life care, and associated mental health challenges. You will engage with clients and their families to provide not only support but also education regarding coping strategies and resources available in the community, thus enhancing their emotional resilience during difficult periods.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The salary range for this position is USD 75,000 – 90,000 per year, commensurate with experience and qualifications.
