Supportive Housing Case Manager – Fairfield, CT Delta-T Group is a staffing agency that connects professionals with client opportunities within the special education, social service, behavioral health, and disability sectors. Client Availability Full schedule client opportunities. Hours: Mon‑Fri 8:30‑4:30. Responsibilities Identify the needs of clients and establish social services support to assist them as they receive their new housing. Identify eligibility for housing and assist with applications for housing vouchers. Assist with finding and securing apartments for clients. Work with the homeless population to screen documents, including sensitive information. Assist with identification and documentation for securing basic needs and entitlements such as Medicaid, housing subsidies, and food subsidies. Initiate referrals for mental health and addiction treatment services as needed. Encourage social supports such as peer activities, groups, and development of community connections. Assist with employment support, including resume writing, job searching, and interviewing skills as appropriate. Continue to provide case‑management activities for newly housed clients after they receive housing, assisting in their continued success. Requirements Bachelor's degree strongly preferred; strong housing case‑management experience also considered. Case‑management experience in supportive or transitional housing is required.
