Job Description & Requirements

Shift: 3x12s Nights 7:00 PM - 7:30 AM Requirement description : · Must have a minimum of 24 months PCU experience (with recent experience in last 12 months) - [REQUIRED]
· Life support certifications must be from AHA, ARC, or ASHI - [REQUIRED]
· No hits on state license to practice - [REQUIRED] [DOCUMENT]
· Flu Vaccination: Medical and/or religious exemption forms available upon request. Please include vaccination status in submission notes.
· Travel experience - [HIGHLY PREFERRED] 
· Active license to practice—verified in last 30 days [REQUIRED] [DOCUMENT]
· Active ACLS certification from approved vendor [REQUIRED] [DOCUMENT]
· Active BLS certification from approved vendor [REQUIRED] [DOCUMENT]
· Flu vaccination [REQUIRED] [DOCUMENT]
· Driver's License [REQUIRED] [DOCUMENT]

 

CERTIFICATION REQUIREMENTS :

• BLS (AHA)
• ACLS (AHA)

STATE LICENSE REQUIREMENTS :

• Arizona

ADDITIONAL LICENSE REQUIREMENTS : Must have active state license to practice at time of submission
