Prevention Family Specialist - Youth & Family Case Manager

Together for Youth seeks a dedicated Family Specialist to assess family strengths, identify needs, and implement treatment and discharge plans. You will collaborate on alternative placements and ensure timely discharge summaries, while maintaining strict confidentiality and coordinating with schools and community agencies.
