All Locums Physicians and NP/PA need to be within 30-45 miles of the center they are supposed to work and willing to cover all clinics in a region and travel up to an hour in a region to cover clinics. 

Practice Setting: Urgent Care
Location: Maryland
Start Date and Length of Assignment: Ongoing PRN Shift (Multiple Locations have openings) Looking for providers to work minimum 6-8 shifts per month
Looking for NPs to work minimum 3 shifts per week, full time due to supervising slot

Schedule: 8am-8pm 
Typical cases seen/required: General Urgent Care - COVID testing, Physicals. Procedures one can expect are: I&Ds of Abscess and/or Paronychia, Suturing, Digital Block, Pelvic Exams, Ingrown toenail removal/Nail removal, Silver nitrate treatment/Electrocautery treatment, Tick Removal, Burn treatment (first, second and third degree), Fluorescein stain/Wood’s lamp exam, Manipulation of dislocated joint, EKG interpretation, Radiology interpretation (over-read by Radiologist), Epistaxis control
Patient volume: 20-40 patients/12 hours (up to 4 patients/hour)
Ages: 2+
EMR: Experity (Velocidoc)
Support Staff: MAs (some clinics have RNs and/or LPNs as well)
Provider will be working solo
Onboarding/Credentialing Time: 1 Week + in person shadow date
Provider complete a short contract packet to sign and then some trainings they need to complete.

Internal Job Details:
MD Bill: $160-$180/hour
NP/APRN: $110-$140/hour (note: $140 is their high side so we consider others at a lower rate first)Travel and Lodging: Lodging available and mileage reimbursement after 60 miles roundtrip
Medicaid/Medicare Number Required prior to presentation: no
Presentation Requirements: Name Clear needed ; NPDB self-query needed at presentation, CRH Locums Presentation form required at presentation plus CV
24-hour response: Y
Interview process: Phone followed but 4 hour paid shadow at a clinic with trainer
Requirements: BC, DEA, NPDB Self Query. Must have clean BG (I.E clean NPDB); ACLS, BLS , DEA number, Provider Attestation questions
Cannot consider anyone with malpractice or discipline history
Provider complete a short contract packet to sign and then some trainings they need to complete. EMR training required – Modules and a 2-4 shadow in clinic (paid)
Must have experience for Urgent care
EMR training required – provided by client. They also like to have the provider come in for shadowing/training (paid) - 4 hours 
New Grads Accepted for physicians. Must have experience in Urgent care

1. Clinic opening time has been confirmed as: (7:30am/8:00am) (YES/NO)
2. The Provider confirms they will arrive at the clinic location 15 minutes prior to clinic opening time. (YES/NO)
3. The Provider agrees to accept all walk-in patients until clinic close time (7:30pm or 8pm). We do not close at that time and the Provider will evaluate and treat all patients who have arrived and checked in prior clinic close time.(YES/NO)
4. The Provider understands that closing the clinic early is not an option unless there is an emergency and this should not be requested. (YES/NO)
5. The Provider (Physician/NP/PA) must remain in the clinic and may not leave the clinic while the clinic is open, during operation hours, unless there is an emergency. They must contact the clinic manager before leaving. (YES/NO)Click or tap here to enter text.
6. This Provider is proficient in procedures (suturing/I&D/digital block) and can read their own x-rays, with no overread. (YES/NO)
7. The provider agrees to complete all chart documentation and sign all patient notes before the end of shift each night.(YES/NO)
8. The provider understands they are responsible for reviewing all labs and x-rays in the resulted queue. This queue must be at zero at the end of their shift each day. Also, they are responsible for calling the patients to discuss all positive lab and x-ray results.
