Position Overview and Qualifications- Oversee all elements related to the provision, facilitation, and enhancement of high-quality clinical care for cardiovascular patients.
- The primary role of the Navigator is to establish strong relationships with patients and physicians, coordinating care plans that encompass appointments, transportation, education, and support services, while representing the patient within a multidisciplinary team.
- On a broader scale, the Navigator will monitor program performance in alignment with organizational goals (growth, quality, practice guidelines, etc.) and identify ways to optimize care practices through the development of evidence-based protocols.
- Act as a bridge between patients and families, primary care physicians, providers, and staff from various healthcare sectors, including specialists and support network members such as social workers.
- This position involves conducting outreach and marketing efforts both internally and externally.
- Follow up with patients via phone within 72 hours post-discharge to evaluate their needs, reinforce discharge instructions, and gather feedback on their care experience.
- Schedule: Monday to Friday with flexible start times before 8 AM; no commitments on holidays, weekends, or after hours; this leadership role collaborates with other cardiovascular leaders and offers potential pathways to supervisory or management positions.
Required Qualifications:
- Graduation from an accredited nursing program; a BSN is preferred.
- Current licensure from the state nursing board is mandatory.
- BLS certification is required upon hire; ACLS certification must be obtained within six months of hire.
- A minimum of three years of experience in cardiology with proven clinical competence is essential.
