Job Description

On-site work location. You are required to work at your assigned work location during your scheduled work times. Please note: Work arrangements are subject to change at any time. About the position: The Nurse I provides foundational psychiatric and medical nursing care within the scope of practice for the LPN license within the MHTL Homes. This position is responsible for executing daily clinical operations, including medication administration, clinical assessments, order transcriptions, and health education. Working as a vital member of a multidisciplinary team, the Nurse I ensures client safety, supports treatment continuity during admissions and discharges, and fosters client independence through targeted health and wellness education.

Medication Administration and Transcription: Responsible for the administration of Long-Acting Injectables (LAIs) and routine medications in strict accordance with the Medication Administration Record (MAR). Responsible for accurately transcribing written physician orders, updating MARs/TARs (Treatment Administration Records) per provider instructions, and conducting regular medication counts. Responsible for coordinating directly with the pharmacy for timely reordering and inventory management.

Clinical Assessment, Documentation and Health Education: Conducts various physical and psychiatric assessments as needed to evaluate client status (e.g. vital signs, basic mental status checks), documents all findings accurately and chronologically within the Electronic Health Record (EHR). Prepares and facilitates client groups focused on health education, medication management, personal hygiene, and infection control.

Care Collaboration, Admissions, and Discharges: Collaborates closely with the Advanced Practice Provider, the MHTL Home clinical team, and the Medical Director to optimize client care. Works in lockstep with the multidisciplinary treatment team to facilitate safe, structured client admissions and discharges. Actively attends and contributes clinical insights during Individualized Service Plan (ISP) meetings when and if available.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

LICENSED PRACTICAL NURSE I (C6T1XX) Education/Licensure/Certification/Experience: Current, valid licensure as a Licensed Practicing Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC). Complete Board-approved IV certification course including supervised clinical; OR document completion of such from another state. Maintains evidence (both LPN and employer) of original documentation and demonstration of IV knowledge/judgment/skill. Follows written IV policies and procedures approved by medical staff, nursing, and agency administration. Advanced Educational Preparation for LPNs in Colorado is limited to IV certification. No substitutions. Work Experience: Hospital settings, including mental health; Department of Corrections; Detox/Rehab Facilities; and medical environments. Preferred Qualifications: Experience working with adults diagnosed with Serious Mental Illness (SMI). Qualified in electronic medication administration records (eMAR). Clinical and psychiatric assessment skills. Knowledge of psychotropic medications and medication monitoring. Experience providing medication education to clients. Experience facilitating psychoeducational or nursing groups. Verbal de-escalation and crisis intervention skills. Knowledge of trauma-informed, recovery-oriented, and person-centered care. Experience working within a multidisciplinary treatment team. Strong clinical documentation and EHR experience. Strong critical thinking, organization, and time management skills.
