Our client, a healthcare organization dedicated to compassionate hospice care, is seeking a Social Worker to join their team. As a Social Worker, you will be part of the Clinical Operations department supporting interdisciplinary team members, patients, and their families. The ideal candidate will demonstrate empathy, strong communication skills, and resilience, which will align successfully in the organization.

Location: Cadillac, MI

Pay Range: $59,055 - $73,842

Shift: Monday - Friday, 8:00 AM - 5:00 PM with flexible scheduling; on-call rotation with additional compensation

What's the Job?

• Collaborate with the interdisciplinary team to provide comprehensive psychosocial assessments for hospice patients and their families.
• Assist patients and families with placement, legal planning, funeral arrangements, and connecting with community resources.
• Evaluate social, emotional, and financial factors affecting patient care and develop tailored care plans.
• Offer counseling and grief support, respecting cultural diversity and individual needs.
• Document patient interactions accurately, ensuring compliance with organizational and regulatory standards.

What's Needed?

• A Bachelor's Degree in Social Work (BSW) from an accredited institution; a Master's Degree (MSW) is preferred.
• Current license to practice social work issued by the State of Michigan.
• At least one year of paid social work experience, preferably in hospice or healthcare settings.
• Excellent communication skills, emotional intelligence, and the ability to maintain confidentiality.
• Proficiency with technology used in clinical practice and documentation.
