Primary Care Nurse Practitioner

Healthcare United is seeking a Primary Care Nurse Practitioner for an outpatient primary care opportunity near Clearwater, St. Petersburg, and the Tampa Bay region of Florida. This is an excellent opportunity for an APRN / Nurse Practitioner interested in adult primary care, chronic disease management, preventive medicine, Medicare Advantage, managed care, and team-based outpatient medicine. The role offers a supportive clinical environment, bonus potential, and the opportunity to build long-term relationships with patients in a stable outpatient setting. The incoming provider will work closely with physicians and clinical support staff to deliver high-quality, patient-centered care.

Job Responsibilities:

• Provide outpatient primary care services
• Manage adult primary care patients
• Support preventive care, chronic disease management, and follow-up visits
• Conduct patient assessments and develop individualized care plans
• Collaborate with physicians and clinical support staff
• Provide patient education and care coordination
• Maintain accurate and timely clinical documentation
• Support quality outcomes and continuity of care
• Help manage chronic conditions including diabetes, hypertension, COPD, heart disease, and other common adult medicine concerns
• Participate in a team-based clinical care model

Requirements:

• Nurse Practitioner / APRN
• Active Florida license or ability to obtain
• Primary care, internal medicine, family medicine, or geriatric experience preferred
• Medicare Advantage or managed care experience preferred
• Adult medicine experience preferred
• Strong patient care and documentation skills
• Comfortable working in an outpatient primary care setting
• Team-based clinical approach
• Strong communication and organizational skills
