We are searching for an experienced Registered Nurse (RN) with a strong background in home health and OASIS documentation for a 13-week full-time contract assignment. Schedule:

• 13-week contract, with opportunity to extend
• Monday through Friday, full-time hours
• On-call availability may be required
• Daily visit expectations ensure a predictable routine

Qualifications:

• Active and unrestricted RN license
• At least one year of recent home health RN experience
• Required OASIS documentation experience
• Proficiency with HomeCare HomeBase (strongly preferred)
• Reliable transportation and valid drivers license
• Confidence managing a full-time caseload and appointments

Responsibilities:

• Deliver skilled nursing care and wellness interventions in patients homes
• Perform thorough patient assessments and document findings with OASIS accuracy
• Implement care plans and collaborate with multidisciplinary clinical teams
• Execute wound care, medication management, education, and chronic disease monitoring
• Communicate clearly regarding patient status and care needs to ensure coordination
