Responsibilities:

• Provide direct patient care to our residents, including administering medications, wound care, and vital sign monitoring

• Collaborate with the healthcare team to develop and implement individualized care plans

• Perform routine tasks such as vital sign checks, ambulating patients, and reporting any changes in patient condition

• Document patient progress and communicate with family members or caregivers regarding patient status

• Participate in quality improvement initiatives to ensure the highest level of care for our patients

Requirements:

• Current LVN license in the state of Texas

• High school diploma or equivalent required; Associates degree in Nursing or related field preferred

• Ability to work a rotating shift, including evenings, nights, and weekends

• Strong communication and interpersonal skills

• Passion for delivering exceptional patient care
