Physical Therapist Case Manager, Level 2: Home Health

Physical Therapist Case Manager – Level 2 Location: San Jose Area (San Jose or surrounding). All roles require field visits to patients’ homes across the Bay Area. Responsibilities Assess patients at home and develop individualized care plans. Implement medically prescribed physical therapy programs. Supervise two Physical Therapy Assistants, ensuring performance standards are met. Coordinate with physicians and interdisciplinary team to maintain quality of care. Document and report patient progress to medical directors. Provide concierge services, including documentation and administrative tasks. Design flexible work schedules; assign zip‑code based territories to minimize travel. Benefits Competitive base salary with incentive bonus program. Qualifications Current unrestricted California Physical Therapist license. Current BLS/CPR certification. Valid California driver’s license. Minimum 1 year of supervising PTAs in a home‑health environment (preferred). Experience in home‑health care and with high‑acuity patients (preferred). Knowledge of local community resources and state/federal home‑care regulatory guidelines (preferred). Strong case‑management fundamentals. Excellent interpersonal, leadership, and critical‑thinking skills. Self‑directed, organized, empathetic, and culturally aware. Additional Requirements Must complete a computer literacy and competency assessment prior to interview. Commitment to follow electronic charting and web‑based communication application.
