Full Time Physical Therapist Case Manager, Level 2 Anticipated Territory: Fremont Area Our clinicians are dedicated to providing skilled services to patients in their homes. As a Physical Therapist Case Manager, Level 2, you will evaluate and treat patients using medically prescribed physical therapy programs, while supervising two Physical Therapy Assistants (PTAs) to ensure high-quality care. Responsibilities Perform home evaluations and develop individualized care plans. Create and manage detailed treatment plans for each patient. Supervise PTAs and monitor their performance standards. Assist patients with mobility and regaining independence. Communicate and coordinate with physicians to ensure quality care. Evaluate and report patient progress to the medical director. Coordinate concierge services and assist with documentation and administrative tasks. Qualifications Current unrestricted California Physical Therapist license. Current BLS/CPR certification. Valid California driver’s license. Knowledge of local community resources is required. Knowledge of individual development and human behavior as they relate to illness/injury and cultural influences on health care. Knowledge of state and federal home‑care regulatory guidelines preferred. Experience in home‑health care is highly preferred. Experience with high‑acuity patients is a plus. Experience working with an interdisciplinary team is highly desired. Case‑management fundamentals are highly desired. 1+ year experience supervising PTAs in a home‑health environment is a plus. Self‑directed, empathetic, and a critical thinker with outstanding interpersonal and leadership skills. Benefits Competitive base pay with incentive bonus program. 401(k) with employer matching.
