We’re currently hiring for a Visiting Nurse to provide mobile primary care in the Blue Ridge area.This is a full-time opportunity with flexibility. Responsibilities: Looking for someone with a Hospice heart and passion for providing end of life care Provide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG Responsible for overseeing patient plan of care with the collaborating physician Provide supervisory visits and oversight for LPN and CNAs Provide education to patients, families, and facility staff Provide oversight to Aides
