Key Responsibilities

• Assessment & Care Planning: Complete and submit accurate Minimum Data Set (MDS) assessments (scheduled and unscheduled) according to federal and state regulations.

• Reimbursement Management: Set Resource Utilization Groups (RUGs) levels to ensure maximum, accurate reimbursement.

• Interdisciplinary Team Coordination: Lead UR meetings, collaborate with nursing, PT/OT, and dietary staff to review patient progress and update plans.

• Quality Assurance: Monitor patient care for quality indicators, ensuring compliance with state and federal regulations.

• Compliance: Conduct interviews and review medical records to ensure all documentation justifies the care provided and reimbursement level. 

Required Qualifications & Skills

• License: Active Registered Nurse (RN) or Licensed Practical Nurse (LPN)

• Experience: Previous experience in LTC/SNF, with a strong understanding of MDS 3.0 and RAI processes.

• Knowledge: Deep understanding of Medicare/Medicaid regulations, ICD-10 coding, and care planning processes.

• Skills: Strong assessment skills, attention to detail, critical thinking, and proficiency in Point Click Care EHR. 

Core Functions

• Conducting assessments: Assessing residents for functional, nutritional, behavioral, and physical needs.

• Updating records: Maintaining MDS records for all residents in the facility.

• Improving care: Working with other staff members to improve patient care.

• Investigating issues: Investigating complaints from family members or residents. 

Required Knowledge

• Regulations: Knowledge of state and federal guidelines.

• Reimbursement: Knowledge of Medicare reimbursement.

• Software: Experience with software for the MDS. 

Daily Activities

• Coordinating care: Leading meetings to develop care plans.

• Documenting: Recording data on charts.

• Evaluating: Reviewing residents' conditions. 

Day-to-Day Tasks

• Assessing patients: Reviewing charts.

• Communicating: Discussing care plans with family.

• Maintaining records: Submitting documentation.

• Attention to detail: Ensuring accuracy in documentation.

• Organization: Managing timelines for assessments.

• Communication: Interacting with other healthcare staff.

• Experience: Previous experience in a long-term care setting.

• Knowledge: Expertise in MDS software and regulations.

• Skills: Exceptional clinical assessment and communication skills.
