Houston, TX 77019Program Manager, SDOH - Job OverviewSchedule: Monday-Friday (8AM-5PM)Are you ready to make a meaningful impact in community healthcare? At Legacy Community Health, we're excited to welcome a passionate individual like you to our team as the Manager of Programs, Social Determinants of Health. This role is your opportunity to contribute to the well-being of our community by enhancing programs that address vital social determinants.Help expand our Health Advocate Student Internship program, Food Insecurity initiative, and Food Prescription program.Collaborate with primary care providers and community-based organizations to support patients in accessing essential resources.Be part of a mission-driven environment where your work directly impacts the health and happiness of the community.Enjoy a supportive, team-oriented atmosphere that values your growth and personal development.Key ResponsibilitiesCommunicate with sites and train staff on workflows for the Food Insecurity Initiative.Maintain and distribute the FI Resource Guide to patients.Attend regional meetings on Food Insecurity and report updates on FI programs.Develop and revise training curriculum related to Social Determinants of Health.Supervise and mentor a team of university-level Health Advocate Student Interns and AmeriCorps Members.Guide Advocates' professional and leadership development.Manage a team of Advocates, lead training sessions each semester, and complete Field Learning contracts and student evaluations.Provide case management support and evaluate Advocates' performance.Communicate innovations and challenges to the Director of Programs.Ensure compliance with Legacy Community Health standards.Provide direct patient care and advocacy.Support the integration of Health Connect programs into the clinical setting.Build and maintain a local network of resource providers.Operate the case management and care coordination database.Maintain confidentiality per Legacy and HIPAA guidelines.Participate in Legacy’s Performance Improvement Program.Travel between sites as needed.Interview and advise patients on health and illness prevention.Minimum QualificationsBSW or MSW from a CSWE accredited institution required.Minimum of 2 years experience in social services in a clinic setting required.Preference will be given to LMSW licensure with 2+ years of direct practice experience, LCSW preferred.Excellent interpersonal and communication skills, both verbal and written required.Good knowledge of Microsoft Office suite.Knowledge of public benefits and community resources.Must manage multiple deadlines and decision-making responsibilities.Reasonable accommodations may be made to enable individuals with disabilities.About Legacy Community HealthAs the largest Federally Qualified Health Center (FQHC) in Southeast Texas, Legacy Community Health has been delivering comprehensive, high-quality, and affordable health care for nearly 40 years.
