Description

• Conduct assessments in either inpatient or outpatient environments and formulate personalized treatment strategies
• Implement interventions aimed at enhancing mobility, strength, stability, and overall function
• Work hand-in-hand with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Guide patients and their families about rehabilitation goals, transition-to-home planning, and at-home exercise routines
• Carefully document evaluations, therapeutic interventions, and patient progress in line with facility procedures and regulatory standards
• Take part in interdisciplinary team meetings and care rounds to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited educational institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (for example, in geriatrics or neurology) are welcome but not mandatory

Skills

• Prior experience is highly preferred
