Overview
Licensed Practical Nurse – Long Term Care (LPN) for a travel assignment in Saint Albans, Vermont. The start date is 11/02/2026, with an 8-week duration and guaranteed 40 hours per week. Weekly pay ranges from $1,033 to $1,082. Shifts are 16 hours long and scheduled for days; the arrangement is local only with weekend coverage, and OT is not guaranteed. This assignment is in a LTC-acute rehab setting and is managed by Healthforce Healthcare LLC.

- Schedule notes: Local only; OT not guaranteed; weekend coverage; typical shift 7:00 AM–11:00 PM Responsibilities
- Provide direct nursing care to residents in a long-term care/acute rehab setting, including assisting with activities of daily living, monitoring vital signs, and supporting medication administration as ordered.
- Document accurately in charting/EHR and ensure proper medication administration, MAR compliance, and timely update of patient records.
- Recognize and report changes in patient condition to the supervising nurse; respond to calls and emergencies following established protocols.
- Collaborate with the interdisciplinary team to plan and implement individualized care plans; participate in rounds and care conferences as assigned.
- Maintain a safe, clean, and dignified care environment; follow infection control practices and fall/wound care precautions as appropriate. Requirements

- Ability to work 16-hour day shifts and weekend rotations
- Availability for an 8-week travel assignment with guaranteed hours
- Comfortable working in a long-term care and acute rehab setting
