Hospitalist Physician

Part-Time Hospitalist - in home setting - Opening in Burlington, MA - Located 17 miles from Boston About the Job:

• Seeking a part-time Hospitalist; 20 hours/week
• Provide high quality, patient centric, cost effective in-patient level of care in the home setting
• Clinical outcomes are equal to or better than inpatient hospitalization.
• Work in a team environment (Physicians, NPs, RNs) in a virtual Command Center
• Medically Home significantly reduces total medicine expense from avoidable inpatient visits, post-acute care and readmissions.
• Improved patient experience.
• State of the art equipment at Command Center and patients Äô home.
• Patients have home infusion, meals/nutrient consults, pharmacy/medical mgmt., mobile imaging, laboratory and DME/oxygen.
• Integrated care with primary care providers, specialists & ancillary departments.
Minimum Qualifications: - Licensure, Certification, Registration: - BC/BE Internal Medicine/Family Medicine/Geriatrics.
• Skills, Knowledge & Abilities:

- Experience as a hospitalist or SNF a plus. - Committed to providing high quality, evidenced based medicine to a diverse patient population. - Excellent clinical and communication skills. - Demonstrated ability to work in a team environment. Location Conveniently located, Burlington, Massachusetts is 17 miles north of Boston and a short drive to the mountains, lakes and seaside of New England as well as cultural opportunities of Boston.
