RN, Admission Hospice - HPH Hospice

Role The Admission RN assesses patients in their place of residence or at hospitals and initiates care into the services of Chapters Health System. The role ensures a seamless transition to Care Navigation and serves as an ambassador of the organization. Qualifications Current RN license in the state of employment. Minimum one year nursing experience; hospice or hospital experience preferred. Previous experience with an EMR/EHR system. Strong clinical assessment skills; background in emergency department or ICU preferred. Excellent communication skills in English; bilingual Spanish preferred. Valid driver’s license and automobile insurance, with reliable transportation. Ability to work in a team environment and manage multiple priorities. Ability to perform essential functions and physical requirements, including lifting patients and equipment, bending, pushing/pulling, kneeling. Strong goal/outcome orientation and time‑management skills. Excellent customer service focus. Active BLS certification from the American Heart Association or Red Cross. Responsibilities Complete clinical admissions for all company services. Explain services to potential patients/families, address questions, and ease concerns in an empathetic manner. Identify admission objections or barriers and provide appropriate responses. Educate patients/families on admission requirements and address immediate needs. Complete informed consents and initial assessments per regulations and company policy. Coordinate with physicians, obtain plan of care, and initiate skilled nursing interventions. Obtain physician orders and complete documentation in line with company policy. Serve as company representative at assigned facilities and facilitate referrals across service lines. Communicate frequently with Care Navigation staff to ensure timely follow‑up; notify team when assignment is complete. Provide clinical communication using SBAR and discuss potential needs with after‑hours staff. Obtain consents for services and arrange safe discharge efficiently while meeting productivity expectations. Collect medical record information for referrals. Build strong relationships with case managers, physicians, and other professionals at facilities. Collaborate with Professional Relations Representatives (PRRs) to educate on available services. Provide care for GIP patients in hospital, including daily visits, documentation, and efficient discharge. During emergencies (e.g., hurricanes), the triage RN may be required to report to an alternate location to ensure continuity of services. Perform other duties as assigned. Physical Demands Key physical abilities required include seeing, hearing, talking, walking, using hands, standing, sitting, reaching, lifting, bending, balancing, and occasionally pulling, pushing, stooping, kneeling, and climbing stairs. Schedule Benefits start Day 110am - 9pm, with rotating weekends. Compensation Pay range: $67,976.53 – $106,213.33. Eligibility & Background Screening All Florida affiliates submissions go through the Florida Care Provider Background Screening Clearinghouse.
