Home Health Physical TherapistPride Health is hiring a Home Health Physical Therapist to support our clients home health patient population across Wabash, Huntington, and Grant County, IN. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!Job Details:Location: Marion, IN 46952Coverage Area: Wabash/Huntington/Grant County May also support patients in Blackford, Delaware, and Madison Counties (as needed)Job Type: ContractShift: Monday–Friday 8:00 am to 4:30 pm (40 hrs/week) Weekend Rotation: Every 3rd SaturdayPay range: $54 to $56/hr on W2 Mileage reimbursement provided (from first patient to last patient visit)Key Responsibilities:Conduct comprehensive patient evaluations and develop individualized physical therapy plans of care in accordance with state guidelinesPerform home health visits, including: Start of Care (SOC)Resumption of Care (ROC)EvaluationsDischargesDeliver skilled physical therapy interventions to patients in their homesComplete all clinical documentation within 24 hours of each visitParticipate in interdisciplinary team meetings, case conferences, and branch meetingsMaintain flexibility in scheduling to meet patient care needsParticipate in weekend rotation and be available for occasional holiday coverageRequired Qualifications:Current, unencumbered Physical Therapy license in the state of IndianaAHA BLS certification requiredValid driver's license, auto insurance, and access to a reliable vehiclePrevious home health experience with understanding of Medicare guidelinesHomeCare HomeBase (HCHB) software experience preferred.Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.
