Overview Physical Therapist – General, Bala Cynwyd, Pennsylvania, United States. Travel - temporary assignment beginning 08/05/2026 for 12 weeks. Responsibilities

• Evaluate patients and develop individualized treatment plans to address mobility, strength, balance, gait, and activities of daily living.
• Implement skilled interventions, monitor patient progress, and adjust the plan of care to achieve goals.
• Document evaluations, treatments, and progress notes accurately; communicate findings to the care team and family/caregivers.
• Collaborate with interdisciplinary team members to coordinate care, discharge planning, and appropriate referrals.
• Educate patients and families on home exercise programs, safety, fall prevention, and activity modification to support independent functioning.

Requirements

• Active Physical Therapist license or eligibility to practice physical therapy in the United States.
• Ability to work in a home health setting and travel to patient homes.
• Strong communication, assessment, and documentation skills.
• Commitment to patient safety, professional standards, and compliance with licensure requirements.
• Willingness to travel within the service area and maintain reliable scheduling for home visits.
