We are currently seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) Minimum Data Set (MDS) Coordinator to join our dedicated team of healthcare professionals. Benefits Competitive Pay, commensurate with experience Regular Employee Appreciation Events to include refreshments and gifts Fun and supportive work environment that encourages teamwork and collaboration Initial uniforms provided Flexible Work Schedule Career Growth and Training Opportunities Paid Time Off and 8 Paid Holidays New health insurance plans to include a prescription plan, FSA, and HSA plans Dental and Vision insurance 401K with employer match Tuition Reimbursement Program Job Requirements Current license as an RN or LPN issued by the State Board of Nursing Experience with the RAI process Strong knowledge of PDPM and State Case Mix time‑weighted reimbursement payment system Knowledgeable of the nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long‑term care Effective verbal and written English communication skills, and customer service oriented Strong attention to detail and accuracy, organizational skills with the ability to prioritize, coordinate and maintain multiple projects with high level of quality and productivity Function independently, and have flexibility, personal integrity, and the ability to collaborate effectively with employees and vendors Demonstrate basic to intermediate skills in Microsoft Word, Excel, PowerPoint, and internet navigation, preferred High level of professionalism with the ability to maintain confidentiality Responsibilities Coordinates the completion of the MDS by all disciplines Maintains and follows a schedule of due dates for all MDSs Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy Completes the MDS and inputs into the computer Verifies that assessments have been transmitted and approved by the State in a timely manner Corrects any rejected records and prepares them for re‑submission Formulates resident care plan along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame Schedules and holds care plan meetings on a regular and timely basis Communicates the care plan to appropriate staff members and notifies supervisor if noncompliance by staff is noted Works with all in‑house and ancillary departments to assure understanding and compliance with the care plan Performs other related duties as directed by the Director of Nursing Services and Administrator #J-18808-Ljbffr Sanstonehealth
