Job Details:

• Nurse Practitioners (APRN) and Physician Assistants provide advanced heart failure consultations within the Skilled Nursing Facility (SNF) setting throughout the East Region
• Scheduled time in the advanced heart failure outpatient clinic located in Norwich, CT
• Assess newly admitted heart failure patients in the SNF setting
• Collaborate with other providers on cardiac care with the goal of reducing hospital readmissions
• Assist in coordinating patient discharge and providing bridge appointments in the outpatient clinic
• Monday through Friday schedule with no call required
• Travel only between closely located sites
• Graduate of an accredited Physician Assistant or Nurse Practitioner program required
• Must be licensed, or eligible to be licensed, as a Physician Assistant or APRN in the state of Connecticut
• Cardiology experience required
• Geriatric board certification preferred
• Newly graduated and experienced providers are supported through on-the-job, classroom, and procedural training
