Job Summary: We are seeking a compassionate and patient-centered Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing adult base primary care team. The ideal candidate will provide comprehensive, high-quality care to patients across the lifespan, focusing on health promotion, disease prevention, and chronic disease management in an outpatient setting.

Position Details: Provide comprehensive primary care services, including assessment, diagnosis, and treatment of acute and chronic conditions to patients 14 years and above Perform physical examinations and interpret diagnostic tests and lab results Develop and implement individualized treatment plans Prescribe medications in accordance with state regulations and clinical protocols Deliver preventive care, including wellness exams, immunizations, and health screenings Educate patients and families on disease prevention, health maintenance, and treatment plans Manage chronic conditions such as diabetes, hypertension, and asthma Collaborate with physicians and interdisciplinary care teams to ensure coordinated care Maintain accurate and timely documentation in the electronic medical record (EMR) Participate in quality improvement initiatives and population health management Adhere to clinic policies, procedures, and compliance standards Qualifications Graduate of an accredited Nurse Practitioner or Physician Assistant program Current and unrestricted NP or PA license in the state of practice National board certification (AANP/ANCC for NP; NCCPA for PA) Current DEA license or ability to obtain Basic Life Support (BLS) certification (ACLS preferred) Minimum of 1 2 years of clinical experience in primary care required Skills & Competencies Strong clinical decision-making and diagnostic skills Excellent communication and interpersonal skills Commitment to delivering high-quality, patient-centered care Ability to work independently and collaboratively within a team Strong organizational and time-management abilities Proficiency with EMR systems Work Environment & Schedule Outpatient primary care clinic setting Call rotation with other providers in clinic Team-based, collaborative environment
