Program Description:
NYC Treat to Quit is a program of Public Health Solutions (PHS) and primarily funded by the New York State Department of Health (NYSDOH). The mission of NYC Treat to Quit is to advance health equity by strengthening medical and behavioral health systems, expanding access to evidence-based tobacco dependence treatment, and empowering communities disproportionately impacted by tobacco-related harm.
Specifically, the objectives of PHS’s current five-year contract with NYSDOH are to 1) directly advance health equity by reducing tobacco-related health disparities and inequities in the catchment area 2) work with medical and behavioral health systems to adopt the Clinical Practice Guideline for Treating Tobacco Use and Dependence (Public Health Service(PHS) Guideline) 3) enable grantees to become well-versed in the most up-to-date medication treatment protocols for TUD 4) educate and advocate for local and state laws, regulations and voluntary policies to reduce tobacco exposure 5) support statewide efforts on data collection, research development and distribution of resources to enhance public awareness, engagement and action on tobacco use, e-cigarettes and cessation.
Summary of Position:
NYC Treat to Quit at Public Health Solutions seeks a Project Manager Health Systems who will identify and lead the implementation of health systems engagement strategies with hospitals, clinics, behavioral health centers and FQHCs, within Brooklyn and Queens. This role supports integration of evidence-based cessation interventions, staff training, EHR prompts, and continuous quality improvement efforts to strengthen clinical systems change. The Project Manager will work closely with the NYC Treat to Quit team and will receive direction from the Senior Health Systems Manager under the guidance of the Program Director. This is a grant-funded position ending April 30, 2031.
Specifically, the Project Manager Health Systems will:
- Understand and implement the NYS Department of Health Tobacco Control Program’s strategic plan as well as support the development and implementation of PHS’ tobacco control strategic plan
- Partner with and provide education, technical assistance, and implementation support, including but not limited to Community Health Centers (CHCs), Federally Qualified Health Centers (FQHCs), NYS Lesbian, Gay, Bisexual, Transgender (LGBTQIA+) Health & Human Services Network and systems that serve people with serious mental illness and/or substance use disorder, and other health care provider organizations
- Collaborate with the NYC Treat to Quit staff to achieve region specific work plan deliverables
Clinical Engagement and Systems Change
- Work with clinical staff to implement and enhance tobacco cessation policies and protocols, including the Ask-Advise-Refer model and other evidence-based interventions
- Collaborate with clinical leadership, quality improvement teams and IT/EHR staff to develop or optimize electronic health record prompts, clinical decision support tools and referral pathways
- Train, coach and provide ongoing technical assistance to health care providers and staff on tobacco screening, brief intervention, and cessation counseling
- Collaborate with hospitals, FQHCs, behavioral health systems and local coalitions to support implementation of tobacco-free policies, strengthen referral pathways and promote integration of cessation services into clinical workflows
- Promote the long-term integration and sustainability of tobacco dependence treatment practices within clinical workflows and organizational policies
Health Systems and Provider Engagement
- Lead and coordinate outreach, training, engagement and education activities with healthcare systems, clinical leadership, behavioral health providers and key stakeholders to support implementation of evidence-based tobacco treatment and systems change initiatives
- Coordinate health system activities, including provider trainings, clinical meetings, quality improvement sessions, and partner-led initiatives that support tobacco cessation education and service delivery
- Assist clinical leadership in monitoring program implementation, progress and performance measures; identify barriers and propose continuous quality improvement strategies
- Support health systems in tracking and using tobacco cessation performance data to improve implementation, identify disparities and strengthen quality improvement efforts
- Utilize traditional and digital communication channels to engage clinical leadership, healthcare administrators and policy makers, including development of educational materials, presentations and written communications such as newsletters and op-eds to support program goal
Program Reporting
- Ensure that outcome measures of the work plan area are achieved, report on activities and outcomes accurately and in a timely fashion
- When appropriate, meet with and engage NYC elected officials and other policy stakeholders throughout the year to support program visibility, alignment, and tobacco control priorities
- Maintain accurate, complete and timely documentation of all program activities and outcomes in required reporting formats. This includes submission of reports through state program reporting systems as well as internal organization tracking and data systems
- Collect, organize and synthesize program data related to outreach, health systems engagement, training activities and cessation-related outcomes to support ongoing performance monitoring and quality improvement
State Coordination and Responsibilities
- Attend and participate in mandatory local, regional, and state meetings and trainings. Anticipated 5 times per contract year.
- Actively participate in state program workgroup meetings
- Along with team members, develop a robust health systems engagement strategy that aligns with organizational goals.
- Participate in the yearly data collection project assigned by the state program
- Remain abreast of current on tobacco control issues and all other duties as assigned
Qualifications and Experience:
- Bachelor’s degree or higher in a related field.
- At least 3 years of experience working on initiatives integrating public health in healthcare and behavioral health systems
- Demonstrated experience with policy and community health systems interventions, health systems initiatives and partnership building
- Knowledge of tobacco cessation best practices, treatment approaches and emerging tobacco control issues a plus.
- Excellent oral and written communication and interpersonal skills.
- Excellent computer skills.
- Ability to establish positive connections and on-going communication with clinical leadership, community leaders and stakeholders
- Demonstrated skills in developing and delivering structured training rooted in adult learning principles and organizing and facilitating initiatives to foster engagement with healthcare entities
- Ability to engage with healthcare partners during non-traditional work times which can include evenings, weekends, and willingness to have a flexible schedule.
- Must have excellent organizational skills and the ability to coordinate multiple tasks, projects, and assignments with competing priorities
- Must be able to work independently as well as be a team member
- Familiarity with New York City communities and neighborhoods strongly preferred
- Must be able to travel in between NYC boroughs, across NYS for participation in mandatory state meetings and when eligible across the nation for participation in trainings and conferences
- This individual must reside in the tri-state area (NY, NJ, CT) by their confirmed start date
