Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. What You'll Do as Physical Therapist

• Conduct comprehensive evaluations and develop individualized treatment plans that empower patients to reach their maximum potential.
• Implement evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.
• Monitor patient progress, adjust treatment plans as needed, and ensure timely and thorough documentation.
• Educate patients and their families on treatment options, rehabilitation techniques, and preventative measures.
• Collaborate with physicians and other healthcare professionals to ensure a holistic approach to patient care.
• Help create a welcoming and supportive clinic environment where patients feel comfortable and motivated

Are you a Physical Therapist that meets the requirements below?

• A graduate of an accredited Physical Therapy program with a valid state license.
