Key Responsibilities Collaborate with medical and therapeutic personnel to devise comprehensive care plans that address the unique emotional and psychological needs of each client. Conduct assessments and develop individualized treatment plans tailored to the preferences and history of clients, ensuring that interventions reflect their specific concerns and goals. Provide ongoing support and education to clients and their families regarding available mental health and social services, guiding them through the complexities of hospice care. Utilize various therapeutic interventions, including individual and group counseling, to facilitate emotional healing and resilience in clients. Maintain accurate documentation of client interactions, assessments, and treatment progress in accordance with agency policies and regulatory guidelines. Stay current with advancements in social work practices and hospice care trends to continually enhance your effectiveness and ensure the highest standard of care. Patient Population / Client Focus In this role, you will primarily work with elderly clients facing terminal illnesses and their families. Your focus will include addressing their emotional and psychological struggles, providing compassionate support, and ensuring that they feel heard and valued during this critical time. You will also work closely with caregivers, assisting them in managing the complex range of emotions that arise in a hospice setting. Qualifications Education: A Master’s degree in Social Work is required to ensure you possess the foundational knowledge of theories and practices appropriate for effective social work. Licensure: An active LCSW license is required to practice in Nevada and to ensure compliance with state regulations. Skills: Excellent communication, interpersonal, and clinical skills are essential, enabling you to build trust with clients and collaborate effectively with the care team. Compassion: A heart for serving those in need, with a focus on enhancing client dignity and quality of life during their final stages. Preferred Experience A minimum of two years of experience working in social services, specifically in a hospice or palliative care setting. Experience with geriatric populations is highly preferred, providing insights into the unique needs and challenges faced by older adults. Compensation and Benefits The salary for this position ranges from $90,000 to $110,000 annually, commensurate with experience and qualifications.
