Licensed Practical Nurse (LPN) specializing in Long-Term Care is needed for a travel assignment in a skilled nursing rehab setting in Middlebury, VT. Start date: 10/01/2026. Duration: 5 weeks. Responsibilities
- Deliver direct nursing care to adult residents in a post-acute/Skilled Nursing Rehab unit, including ADL assistance and mobility support
- Monitor and document vitals, provide timely responses to patient needs, and escalate changes in condition per protocol
- Administer medications and IV therapies using IV pumps as prescribed, and perform basic phlebotomy and EKGs as indicated
- Document care activities in the electronic medical record (EPIC) and ensure accurate, complete charting
- Collaborate with LPN/LNA/CNA teammates, unit clerk, and supervisor to coordinate patient care and ensure safe transitions
- Adhere to dementia-capable care standards, infection control, and facility/unit procedures; participate in rounds and care planning as assigned

Requirements
- Licensure: active Vermont Licensed Practical Nurse (LPN) license at time of submission
- Experience: minimum 1 year clinical experience; prior travel experience preferred
- Certifications: BLS required; IV certification preferred
- Technology: EPIC experience within the past 2 years required
- Background checks: must complete a 7-year nationwide background check (including county court searches, FACIS3, and sex offender registry) within 18 months of start; FACIS-3 and VCIC checks within 30 days of start
- Special qualifications: dementia-capable care certification preferred; able to work all holidays as required
- Additional: EPIC and medication administration experience; able to work in a post-acute / LTC setting; demonstrate reliable transportation given local logistics
