Assume responsibility for caseloads consisting of patients in specialty areas Conduct initial assessments to obtain thorough physical, social, and environmental data Establish plans of care according to physician orders and patient needs Provide direct patient care and adjust nursing care processes as needed Identify problems, modify nursing care plans, and evaluate outcomes Advise and consult with families and other personnel Assess patient responses to treatments and teaching and apply interventions Update and revise plans of care Complete visit assessments, OASIS, 485, and verbal-order documentation accurately and promptly Communicate with physicians to confirm and update patient plans of care Provide status updates and maintain communication with physicians and healthcare-team members Facilitate discharge planning and assure continuity of care after discharge Participate in developing discharge plans with patients, families, physicians, social workers, and staff Participate in chart audits and continuous quality-improvement activities Identify, develop, and participate in process-improvement opportunities Maintain and update accurate clinical and patient records according to agency, state, and federal guidelines Provide age-appropriate care based on patient assessment and departmental policies and procedures Requirements Associate's Degree in Nursing Registered Nurse license issued by the state in which the team member practices Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required A valid driver's license issued by the Division of Motor Vehicles, or driver license issued by the state of Illinois Typically requires 3 years of experience in clinical nursing Excellent communication skills Strong interpersonal skills and ability to work with a variety of clients with patience and respect Good organizational, analytical, and problem-solving skills Ability to work well with physicians and other professionals Proficiency in clinical skills and ability to work under direction and make sound judgments Ability to operate FAX, computer, copier, and telephone system Ability to assess and interpret patient-status data Tolerance for differences and appreciation of multiculturalism and diversity Proof of a valid driver license and a registered, functional car with adequate insurance coverage Physical ability to bend, twist, kneel, squat, reach overhead, sit, stand, walk, climb stairs, and work in confined spaces Ability to safely work with people of various sizes, weights, medical conditions, and physical limitations Physical stamina to attend to patient care requirements for three or more hours without complete rest Accurate vision to read gauges, measuring devices, and written instructions Ability to lift up to 50 lbs from floor to waist Ability to lift up to 20 lbs from waist to overhead Ability to lift and carry up to 40 lbs at waist height Ability to push/pull with 30 lbs of force Ability to perform a sliding transfer of 150 lbs with a second person present Core Competencies Demonstrates expertise in patient assessment, care planning, and direct patient care while ensuring compliance with clinical guidelines and effective communication with healthcare teams.
