Case Manager ACT Harmony Team $2500 retention bonus Department: TPC-Adult Services Site. Operating Unit The Providence Center Location: Providence, RI Job ID: 20319 Job Status: Full Time Shift: Days Schedule: Other This position is eligible for a $2500 retention bonus. Job Summary The Providence Center Case Manager ACT Harmony Team will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric symptoms. Duties and Responsibilities Coordinate a variety of services: Provide supportive counseling, problem solving, and limit setting. Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with people who have severe persistent mental illness and/or substance abuse issues, as it relates to case management. Monitor client status during spend down process, complete necessary DHS applications. In cooperation with Managers or Emergency Services Staff, assist with assessments, crisis interventions and risk management as needed. Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves. Transport consumers in own vehicle to planned activities and appointments. Complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by TPC confidentiality policy. Maintain cooperative relationships with Center staff, consumers, community agencies and the public. Attend mandatory in-service trainings and other trainings required. Attend case presentations and conferences. Requirements Associate's degree required. Bachelor's degree from accredited four-year College or University in Human Services or related field preferred. Experience in Human Services field preferred. Must have valid driver's license and insured auto. Must be able to work flexible hours.
