Position Summary

The RHTP Grants Management & Implementation Specialist provides grants management, technical assistance, and subject matter expertise to recipients of Moda Health’s Alaska Rural Health Transformation Program (RHTP) grant. This role serves as the primary liaison between Moda Health, funded partners, and other stakeholders to support successful implementation of grant activities. The position is responsible for grant monitoring, compliance, and reporting. This includes ensuring adherence to federal and state requirements and supporting grantees in achieving program goals. The position will also support the development and delivery of training and technical assistance to funded health system partners to implement National Committee on Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) standards and build partner readiness to participate in advanced value-based payment (VBP) models. Working closely with Moda Health’s internal grant and quality teams and external clinical and community partners, the position supports program implementation, reporting to funders, grant proposal development, and continuous quality improvement.

This is a full-time position. The ideal candidate will reside within commuting distance of our downtown Portland, Oregon office or in Alaska’s Anchorage/Kenai Peninsula region. Occasional travel to Alaska and a hybrid in-office schedule is required for Portland-based candidates. This is a limited duration position contingent on continued receipt of RHTP grant funds.

Pay Range

$80,000 - $87,318.58 annually (depending on experience)
Actual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.

Required Skills, Experience & Education:

1. Minimum Bachelor’s degree required. Master’s degree in Public Health, Public Policy, or Healthcare Administration-related field preferred, or an equivalent combination of education and relevant work experience.
2. 3 - 5 years of experience working in healthcare, public health, or health-related community-based organizations. Experience working in or with rural or underserved communities preferred.
3. 2 - 3 years of experience in grant management, including contract management, grant monitoring and reporting, proposal development, and providing technical assistance and support to grantees.
4. Familiarity with NCQA PCMH and/or Oregon’s Patient-Centered Primary Care Home requirements and recognition processes.
5. Demonstrated understanding of VBP systems and alternative payment models (APMs) in healthcare settings.
6. Knowledge of primary care clinic functions such as case management/care coordination, use of electronic health record (EHR) systems, population health-level metrics and reporting, behavioral health integration, and collaboration with Traditional Health Workers (THWs).
7. Proficiency in Microsoft Office, especially MS Excel and other reporting software.
8. Strong verbal, written and interpersonal communication skills.
9. Experience convening diverse stakeholders and facilitating productive virtual and in-person meetings.
10. Ability to manage complex projects, track activities, and meet short and long-term project milestones.
11. Ability to maintain confidentiality and project a professional business image.

 

Primary Functions :

1. Grants Management & Compliance (30%)

   • Serve as the primary point of contact for grantees and ensure alignment of grant activities and reporting with federal, state, and contractual requirements
   • Monitor and track grantee performance and deliverables and ensure timely submission of required financial and programmatic grant reports.
   • Develop and improve grants management policies, processes, and tracking systems.
2. Grant Reporting & Evaluation (10%)
   • Oversee data collection, validation, and analysis for grant-related activities and outcomes.
   • Monitor and evaluate program performance against established metrics and goals.
   • Use data insights to identify and recommend improvements to programs, processes, and policies.
   • Prepare and submit required grant reports to the Alaska Department of Health and other stakeholders.
3. Program Implementation & Technical Assistance (40%)
   • Serve as a subject matter expert on RHTP grant requirements and patient-centered care models, including NCQA PCMH. 
     
   • Collaborate with internal teams to develop and deliver training, tools, peer learning opportunities, and technical assistance to support partner implementation of NCQA PCMH standards.
     
   • Provide grants management and programmatic technical assistance and support to grantees.
     
   • Regularly meet with grantees to monitor implementation progress, identify barriers, and support continuous improvement efforts.
     
   • Recommend and support strategies to address implementation challenges and improve program outcomes.
4. Partner Engagement (20%)
   • Participate in and facilitate stakeholder meetings; develop agendas, presentations, and maintain meeting documentation.
     
   • Build and maintain strong relationships with state agencies, providers, and community organizations.
     
   • Support strategic planning with Moda leadership and stakeholders to strengthen future funding opportunities.
5. Other Responsibilities 
   • Seek out and participate in professional growth and development activities.  
   • Other duties as assigned.

 

Working Conditions & Contact with Others:

• Office environment with extensive close PC and keyboard use, constant sitting, and frequent phone communication. Must be able to navigate multiple computer screens. A reliable, high-speed, hard-wired internet connection required to support remote or hybrid work. Must be comfortable being on camera for virtual training and meetings. Work in excess of standard workweek, including evenings and occasional weekends, to meet business need. Occasional and overnight travel to Alaska may be required.

• Internally with all Moda departments. Externally with partner healthcare organizations and vendors.
