You will provide relationship-based primary care, respond to acute needs, and drive measurable improvements in health outcomes—all with full support from Bloom's interdisciplinary team and advanced data systems. This is the ideal role for a clinician who thrives on independence, enjoys complex primary care, and wants to practice medicine the way it was meant to be: patient-centered, unrushed, and deeply impactful.What You'll DoServe as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.Utilize Bloom's integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.What's in It for You A manageable, quality-focused scheduleMonday - Friday8-12 patients per day (no unrealistic volume expectations)Light call rotation: only 2–3 times per yearWeekends off and flexible daily workflowsRobust benefits – starting Day 1100% employer-paid Health, Dental & Vision effective Day 13 weeks PTO + 7 paid holidays401(k)Employer-paid Basic Life & AD&DCME allowance + paid licensing & malpracticeStrong support from MDs, triage, schedulers, IT, billing, and marketing—so you can focus on patient careMeaningful autonomy + strong support You practice independently, but never alone.
