Position Summary:

To nurture and support the Hawthorne elders by providing excellent quality care in a safe and closely associated family environment. Must be a self-motivated team player that is flexible, creative, energetic and open to learning a new way of enhancing the quality of life for elders. Good verbal and written communication is necessary.

Qualifications & Education Required:

• High school or GED required. 
• Minimum of 2 years work experience in home health, long term care, or other medical setting
• Excellent verbal and written communication skills.
• Must be a self-motivated team player that is flexible, creative, energetic and open to learning a new way of enhancing the quality of life for elders.
• Willingness to participate in Care Partner (RCA) onsite training.
• Ability to interact with elders and care for their needs

Position Competencies:

• Display team work and cooperation with fellow RCAs and LPNs to ensure exceptional care for our elders.
• Display excellent verbal and written communication skills. 
• Proper use of communication tools (pagers, walkie-talkie) to ensure timely response to elder requests for assistance.
• Proper use of electronic devices for recording activities of daily living.
• Excellent attendance and understanding of the impact of calling off.
• Responsible for maintaining a clean and safe living environment for the elders.
• Interpersonal skills: Display appropriate emotional, physical, and social support to ensure the well-being of elders. Display professionalism with co-workers at all levels.
• Work independently and with minimal supervision. 
• Follow departmental policies and procedures.

Position Responsibilities:

• Provide personal care such as dressing, grooming, transfer, assistance with ambulation, toileting to assist elder to maintain good personal hygiene in accordance with the Individual Service Plans (ISP’s).
• As necessary, assist elders with positioning and transporting in and out of chairs, beds, showers, cars, buses, etc. Will also include transporting to other areas of the community.
• Look for daily opportunities to establish meaningful relationships with the elders by valuing and encouraging individuality. 
• Report any observations or changes in residents’ behavior, activity or physical appearance, mental status changes or concerns to LPN, RN/Case Manager or Administrator.
• Encourage independence of elders with decision making.
• Encourage elders to participate in Social Recreation programs and activities.
• Provide redirection to confused or distressed residents with patience.
• Assist in maintaining clean common spaces and laundry rooms. Assist with resident room cleaning as directed by Administrator, RN Case Manager or LPN.
• Attend department meetings as scheduled. Attend quarterly organization wide meetings.
• Complete monthly training, annual mandatory in-service training, bi-annual First aid and CPR training. Participation in additional corporate sponsored training programs is encouraged.
• Communicate appropriately with co-workers at all levels to meet the needs of the elders. 
• Maintain a safe, hazard free environment by observing and monitoring the community and reporting unsafe conditions to the RN/Case Manager or Administrator.
• Follow procedures for documentation.
• Ability to work with individual residents or groups in special activity programs.
• Respond to common inquiries from residents, resident’s families, regulatory agencies, and the public. When unsure of the answer, escalate the inquiry to RN Case Manager or Administrator.
• Maintain a high level of confidentiality with staff and peers. Follow HIPAA guidelines.
• Ability to remain calm, reassuring, and effective in an atmosphere that often times is chaotic.
• Follow established protocol for accidents and incidents.
• Complete additional tasks as requested by Administrator, RN/Case Manager, Project Coordinator or LPN.
• Responsible for following infection control guidelines.
• Attend and participate in disaster preparedness training and drills.
• Assist residents during meals with direction, cueing and assistive devices.
• Monitor elders during meals and track meals on the roster.
• Provide assistance to elders with medical equipment.
• May be required to do other duties as necessary.

Requirements:

Physical Requirements:

• Standing, walking, sitting, climbing stairs, lifting 50 pounds or more, and ability to assist elders with transferring and all ADL’s.

Exposure to Conditions:

• May be exposed to infectious conditions.
