Registered Nurse (RN) - Transplant

Transplant Coordinator Nursing

Schedule: 12-hour variable shifts, MondayFriday, 7:30 AM 5:00 PM; no major holidays or weekends

Requirements:

• 3+ years of transplant coordinator experience preferred
• Strong ambulatory nursing experience required
• Experience coordinating pre- and post-transplant care, reviewing labs, patient education, triage, multidisciplinary coordination, and managing medically complex patients
• Experience with kidney transplant population and Epic preferred
• BLS certification required

Roles & Responsibilities:

• Screen patients for transplant eligibility and coordinate necessary testing and appointments
• Provide patient education regarding pre- and post-transplant care
• Manage medication protocols for transplant patients
• Collaborate with multidisciplinary teams to ensure comprehensive patient care
• Monitor and follow up on patients' clinical progress throughout their care journey
