You will interact with clients primarily via telehealth platforms, making effective use of technology to conduct assessments, therapy sessions, and follow-up calls. While you will have the autonomy to manage your schedule, occasional in-person gatherings with the interdisciplinary team may be necessary for training and collaboration.

Key Responsibilities\n

The primary responsibilities of the Licensed Clinical Social Worker include:

• Conducting thorough assessments of seniors’ psychosocial needs to understand their emotional, mental, and social well-being.

• Developing individualized care plans in collaboration with clients and their families to address their specific goals and aspirations.

• Providing counseling and emotional support throughout various life transitions such as retirement, loss, or health-related challenges.

• Advocating for clients’ rights while assisting them in accessing essential community resources to enhance their care.

• Working closely with an interdisciplinary team comprising health professionals to coordinate services that optimize client care.

• Performing regular follow-up visits, either through telehealth or in-person, to monitor client progress and address any safety concerns in their living conditions.

• Facilitating constructive communication among family members involved in care to resolve conflicts and support a cohesive care approach.

• Educating clients and their families on available community resources, programs, and support groups to enrich their social engagement.

• Maintaining complete and accurate documentation of client assessments, care plans, and progress notes according to organizational policies.

Qualifications\n

• A Master’s Degree in Social Work (MSW) from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
