LPN Continuing Care

As a continuing care LPN, providing top-notch patient care is paramount. Under the direction of an RN, mid-level Provider, or Physician, you will be responsible for assessing, planning, implementing, and evaluating patient care. You will be a vital part of our team, making a real difference in the lives of our patients and their families. You will collect and monitor assessment data and report physiological, psychological, sociocultural, and spiritual needs of patients and families in coordination with the RN, mid-level Provider, or Physician. You will provide direct patient care under the direction of the RN, mid-level Provider, or Physician, consistent with policies and procedures. You will inform patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate. You will document care provided via computer or written patient records. You will greet and communicate with patients and families in a manner consistent with caring and respect. You will communicate and collaborate with other members of the health care team to ensure continuity of care and coordination of service. You will clean the patient's environment, work area, and equipment and communicate any equipment malfunctions to appropriate personnel. You will document patient chargeable items according to standards. You may participate in selected quality monitoring activities under the direction of the RN, mid-level Provider, or Physician. You will demonstrate a high standard of moral and ethical behavior. You will recognize appropriate and ethical behaviors and identify resources for resolution as needed. You will advocate for the privacy, confidentiality, and security of patient, staff, and organization data. You will actively participate in safety initiatives and risk mitigating measures as required. You may perform other duties as assigned.

Experience: Minimum 2 years License: LPN License Certifications: Basic Life Support (BLS) Must-Have: - High School Diploma or equivalent - Graduate of an approved nursing program - Successful completion of a competency-based orientation program - State of Michigan LPN License - State of Michigan Driver's License - Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross
